These help me prepare a personalized and detailed review before our meeting. Forms must be submitted at least 24 hours before the session.Â
3. Meet via secure video
Your live appointment is 30 minutes, but includes 2–3 hours of total professional work, including a full medication review, safety checks, analysis of routines, and development of a personalized action plan to support you and/or your loved one’s health and your peace of mind.
